The short version
- You confirm your licence to practise in The Bahamas is valid and current.
- You confirm you are in good standing.
- You will tell Ionyx within 2 working days of any change, suspension or condition.
- We ask you to confirm this once a year.
1. The statement
I confirm that:
- I hold a valid, current licence or registration to practise my profession in The Bahamas.
- The licence details I gave Ionyx are true and complete.
- My licence is not suspended. It has no condition or limit on the care I offer through Ionyx, except any I have told Ionyx about in writing.
- I only give care through Ionyx that is within my licence, training and competence.
- I will tell Ionyx in writing within 2 working days if my licence ends, lapses, changes, is suspended or has a condition added.
2. How to tell us
Write to Ionyx Support from your account or email techsupport@ionyxai.com. Upload any new licence on the Licence and training page.
A working day is a day that is not a Saturday, Sunday or public holiday in The Bahamas.
3. What happens next
- While we review a change, your profile may be hidden from patients.
- Patients you have already booked may need to be moved to another provider.
- Not telling us is a serious breach of the provider agreement.
4. Once a year
We ask you to tick this statement when you join and once a year after that. We keep a record of each tick, with the version and the date. We may check your licence with the body that issued it at any time.
