The short version
- A provider can write a sick note for work or school after a visit that really happened.
- By default a sick note does not show your diagnosis or the reason you were off.
- You choose whether the reason is added. Your provider writes the words.
- Each sick note has a code. An employer or school can check it is real on the Ionyx website.
- The check shows only what the note shows. It never shows your health record.
- Employers and schools must keep the note private and must not use the code for anything else.
1. What an Ionyx sick note is
A sick note says that, in your provider's opinion, you should stay off work or school for medical reasons on certain days.
- Only a provider can write it. They can only write it after a visit that has started or happened.
- The first day off must be within 3 days of the visit.
- One note can cover up to 30 days. For longer, you need a new visit.
- Your provider is responsible for what the note says.
- A sick note is never changed. If it is wrong, your provider withdraws it and can write a new one.
2. What a sick note shows
A sick note shows:
- your name
- whether it is for work or school
- the first and last day off and the number of days
- the date of the visit
- the provider's name and type
- the date it was written
- a verification code and a QR code
By default it does not show your diagnosis or the reason you were off.
3. Adding the reason (your choice)
Some employers or schools ask for a reason. You decide whether one is added.
- If you want a reason shown, tell your provider.
- Your provider writes the words. They may keep it short, for example "a stomach illness".
- Anyone who checks the code will see the reason too.
- If you change your mind, ask your provider to withdraw the note and write a new one without the reason.
4. Getting and sharing your note
- When your provider writes a note, we tell you. Open it from Visits in Ionyx.
- You can print it, save it or show it on your phone.
- Give the note or its code only to the people who need to check it.
- When you share a note from Ionyx, we ask you to confirm that you choose to share it with your employer or school.
5. For employers and schools: how to check a note
- Go to ionyxai.com/symptom-checker/verify/ or scan the QR code on the note.
- Enter the 10-character code printed on the note.
- The page tells you whether the note is real or has been withdrawn.
The check shows:
- the person's first name and last initial
- work or school
- the first and last day off and the number of days
- the reason, only if the person chose to add one
- who wrote it and the date it was written
- the date it was withdrawn, if it was
The check never shows anything else about the person's health. Checking a code is free and you do not need an account.
After 20 wrong codes in 15 minutes, the check pauses for 15 minutes.
6. What employers and schools may do
- Check that a note given to them is real.
- Keep a copy of the note in the person's file, as long as it is needed.
- Ask the person directly if they have a question.
7. What employers and schools must not do
- Check a code they were not given.
- Try to guess codes.
- Contact Ionyx or the provider for health details about the person without the person's written agreement.
- Pressure the person to add their diagnosis to the note.
- Share the note or its code with people who do not need it.
- Use the note for anything other than confirming the time off.
- Treat the person unfairly because of the note, where the law forbids it.
Health information is sensitive personal data under the Data Protection (Privacy of Personal Information) Act, 2003. If you hold a copy of a sick note, you are responsible for protecting it.
8. Withdrawn and fake notes
- A provider can withdraw a note, for example if a date was wrong. Anyone who checks the code then sees that it was withdrawn and when.
- If the details on a paper note do not match the check, do not rely on the paper. Ask the person about it.
- Changing or faking an Ionyx sick note may be a crime. Tell us at techsupport@ionyxai.com if you think a note is fake.
9. Other Ionyx documents
Other documents from Ionyx providers, such as visit summaries or referrals, may also carry a verification code. The same rules apply to them.
